I acknowledge that my involvement and that of my family members in any club activity or program is purely voluntary. Therefore, I authorize myself and my family to participate in these activities or programs, while understanding that there are potential inherent risks involved.
I shall hold harmless this local Mothers’ Club and any Mothers’ Club volunteers or representatives, paid or unpaid, and/or the providers of any activity or program location and/or materials from any liability and/or responsibility for any physical/psychological accident, illness or injury to myself or any member of my family that occurs during or as a result of any function or program. I understand that I am ultimately responsible for the safety of myself and my family.
